Non-contrast T1 mapping characterizes the myocardium beyond that achieved by late gadolinium enhancement in both hypertrophic and dilated cardiomyopathy

چکیده
Background Both HCM and DCM exhibit variable clinical outcomes. Unfortunately, our ability to risk stratify and characterize disease mechanisms in these patients remains poor, and improved biomarkers are needed. Myocardial fibrosis, both focal and diffuse, is a feature of both HCM and DCM and focal myocardial fibrosis, as assessed by LGE, has been recently identified as a predictor of cardiac death in HCM and DCM. We recently developed a novel method for non-contrast CMR T1 mapping, a recognized marker of increased water content such as in fibrotic tissue. We hypothesized that T1 mapping offers an alternative to late gadolinium enhancement (LGE) techniques, providing additional noninvasive diagnostic information in HCM and DCM.

Myocardial tissue characterization using magnetic resonance noncontrast t1 mapping in hypertrophic and dilated cardiomyopathy.
BACKGROUND Noncontrast magnetic resonance T1 mapping reflects a composite of both intra- and extracellular signal. We hypothesized that noncontrast T1 mapping can characterize the myocardium beyond that achieved by the well-established late gadolinium enhancement (LGE) technique (which detects focal fibrosis) in both hypertrophic (HCM) and dilated (DCM) cardiomyopathy, by detecting both diffuse...
متن کاملSize of myocardial fibrosis by LGE, pre- and post-contrast T1 and ECV in patients with hypertrophic cardiomyopathy referenced to normal appearing myocardium or healthy volunteers
Background Myocardial fibrosis is associated with abnormal cardiac remodeling and poor prognosis in patients with hypertrophic cardiomyopathy. Currently, new cardiac MRI (CMR) techniques such as T1-mapping and extracellular volume (ECV) measurement are available to quantify diffuse myocardial fibrosis. We analyzed the size of myocardial fibrosis using preand post-contrast T1, ECV and late gadol...
متن کاملCan a MR Imaging Scanner Accurately Measure Hematocrit to Determine ECV Fraction?
F ocal fibrosis, most due to myocardial infarction, is readily identified by cardiac magnetic resonance (CMR) using widely validated late gadolinium enhancement methods. However, many common conditions of the heart, ranging from hypertrophic cardiomyopathy to idiopathic dilated cardiomyopathy or myocarditis, are histologically characterized by diffuse, interstitial myocardial fibrosis. In these...
متن کاملQuantitative of myocardial extracellular volume fraction improves characterization of fibrotic burden in patients with apical hypertrophic cardiomyopathy beyond visual assessment with late gadolinium enhancement
Background Apical hypertrophic cardiomyopathy (APH), a variant of hypertrophic cardiomyopathy, is generally considered to have a benign clinical course, with localized hypertrophy and evidence of myofibril disarray and fibrosis at the apex alone. However, traditional late gadolinium enhancement (LGE) techniques are not sensitive enough to detect subclinical apical fibrosis. Accordingly, we hypo...
متن کاملNon-invasive measurement of myocardial extracellular volume using T1 mapping as a novel biomarker of diffuse fibrosis in dilated cardiomyopathy
Background Diffuse myocardial fibrosis is frequently observed in histologies of dilated cardiomyopathy (DCM). T1-mapping techniques have the potential to measure fibrosis quantitatively and therefore provide novel cardiac magnetic resonance (CMR) insights into myocardial tissue characterization beyond the presently employed late gadolinium enhanced (LGE) techniques. Fibrosis results in expansio...
